Dennis Kelly, R.N.

Administrator

After 16 years in finance and management in the Texas financial markets, Denny married a wonderful woman who was in nursing school. Motivated by her example, he returned to school to pursue a career in Nursing. Upon completion of his BSN, Denny worked for a short time in SICU before finding his niche in Hospice. Denny’s association with hospice began 20 years ago and has taken on a variety of roles: RN Case Manager, After-Hours Supervisor, Patient Care Coordinator, Administrator and Clinical Supervisor. Combining the two distinct career paths has merged well through start-up operations in Texas and now leading Horizon Home Health and Hospice as Administrator of Idaho’s premier Home Health and Hospice organization dedicated to providing Life Changing Service to the citizens in Idaho. As an HPNA approved educator, Denny continues to be actively involved with State and National organizations providing education related to health care delivery in non-traditional settings.

Sandy Hamilton, RN, BSN, M.Ed., CHPN

Hospice Coordinator

An Idaho native who grew up in the southwest corner of the state, Sandy received her nursing and education degrees from Boise State University.  Sandy has been a nurse in Intensive Care, a cardiac nurse in a Cardiology practice, a Regional and then Corporate Nurse Consultant, a Charge Nurse for a Medical Surgical Unit, but states emphatically that she “found her true home in Hospice.”  “To be able to help the patient and family in the last phase of this life; to be able to help the whole family obtain closure and complete those final wishes; to create a comfortable and peaceful environment where the patient can stay with family, and the family understands what is happening and creates a final memory of their loved one comfortable and at peace–ready to take the next step in their journey; for me this is not just a job.  It’s a calling and an honor.”

Craig Esplin, PT, MBA

Therapy Coordinator

Craig was born and raised in St. George, Utah.  Craig graduated from University of Utah Physical Therapy program in 1985.  He immediately went to work in a private physical therapy practice where he remained until 1990.  Craig then took a position as Outpatient Therapy Director for HealthSouth Rehab and worked there until 1995.  He received his MBA degree from the University of Phoenix in 1995.  After that he became the Area Rehab Director for Kindred Healthcare, where he remained until 2010.  In 2010 Craig found his new home as Therapy Coordinator for Horizon Home Health and Hospice.  Craig is married and the proud father of four children.  His hobbies include running, racquetball, and gardening.  Craig says, “The reason I’m now at Horizon has to do with professional growth.  It was an opportunity to enter into the home health arena, which I had not been familiar with in my earlier positions.  Horizon offered me a great chance to be a part of a growing, dynamic company with local roots.  It’s the wonderful professionals I get to rub shoulders with that keep me here, and keep me excited about the future.”

LeeAnn Mascroft, RN

Meridian Branch Manager

LeeAnn was born and raised in Sacramento, California, and returned to school after her own children were in high school to begin a career in nursing.  She began by completing her CNA certification and working at a skilled nursing facility in Boise, Idaho.  She then attended Boise State University and received an associates degree in nursing as an LPN.  She has 10 years of experience in hospital rehabilitation nursing, and has been working with Horizon for the past 11 years.  She began as an individual case manager, and now serves as our Meridian branch manager.  “I chose the home health field because of the difference it makes in the lives of our clients.  I learned quickly that there is a very significant impact a nurse can have in the homes of people who are not only sick, but sometimes just lonely, or debilitated.  I love being there with them, in their home, to assist them in a time of sickness or crisis.  I really believe in what I do, in keeping our clients independent in their homes, and maximizing their health potential.  It’s a great place to work!”

Denise Sorensen

Emmett Branch Manager

I come from a family with four brothers and one sister.  I grew up on a large farm in Southeast Idaho and had many responsibilities both in the home and on the farm.  I  now have a husband and three children.  I am on the Little League Board in Emmett and I’m also a member of the Emmett Kiwanis club.

I came to Horizon with a background in the medical equipment field, but no background in home health or hospice.  I stayed with Horizon because of the many experiences I have had that have literally changed my life.  I know that with every phone call I take or with each person walking through my door, there is someone in need of help.  I lost my brother to Muscular Dystrophy and our family was his “hospice agency.”  We did not know about hospice then.  It is a very humbling experience to be able to walk through such a delicate and special time of a family’s life with them.  Having had the experience of losing a loved one and seeing the toll it can take on a family gives me great insight to what families facing similar circumstances may need.  I feel truly blessed to be able to be a part of this agency.

In the midst of what many characterize as a national health care crisis, this group represents a local level solution to local needs and concerns. The people calling the shots for the delivery of care in your community also live and work alongside you in your community. You will deal with them, not some distant administrator with no feel for how things work in your town. As we continue to expand our service coverage area, more local leaders will be added to our team to serve you where you live according to your needs, desires and concerns.
