life. Hospice services support you in giving those you love the
best you can offer in end-of life care. Hospice recognizes death
as the final stage of life. We enable hospice patients to live

Horizon Hospice provides comprehensive and practical end-of-life
care that supports both the patient and the family. Our emphasis
is on protecting the dignity of the patient while offering the
best comfort care available. We offer a holistic approach in the
services we provide.
Care is provided by a team of
qualified caring professionals
- Routine
visits in your home by nurse case managers to provide pain
control and symptom management. A nurse is always available
on-call for crisis management.
- Nurse’s
aides are available to provide personal care and family
support.
- Horizon
Hospice makes every effort to treat body, mind and spirit as
a whole. To this end, we are staffed to provide you with
spiritual services that are non-denominational and
non-judgmental. We also help connect families with their
choice of spiritual support.
- Our social
workers are skilled in a number of areas that include
securing community resources, assisting in financial issues,
facilitating completion of advanced directives, and
providing initial counseling.
- Bereavement
support is offered to support families while their loved one
is on hospice care and for 13 months following the death. We
also provide education and ongoing bereavement support on a
community wide basis.
- Volunteers
offer respite care, patient companionship and are also
available for errands, transportation assistance, and other
care.

In addition to our interdisciplinary
team of professionals and volunteers, Horizon Hospice also
supplies medications for pain and symptom management related to
the terminal illness, and medical equipment needed for comfort
in the home setting.
