Bereavement

The Horizon Hospice bereavement program consists primarily of a thirteen month post death follow-up for the families of our patients. This may include phone calls, visits, and mailings. The frequency and types of contacts for the families are tailored to meet their individual needs. Through our bereavement program families have access to hand-outs, bibliographies, books, and videos that address their specific bereavement concerns.

In addition to the services for the families of our patients, we offer a group on the third Thursday of each month that is open to the public. Grief classes for specific groups and topics are available at different times throughout the year. We are also happy to make referrals to other bereavement services and to private counselors as needed.
