About Us:
Horizon Home Health & Hospice, Inc. is a local, privately held corporation, established by Marcella Little in 1993 in Caldwell, Idaho. In 1994, the home office was moved to Meridian, and has remained there since. Branch offices are located in Caldwell, Mountain Home, and Emmett.

Services Offered:
Home Health and hospice Nursing, CNA, medical social services, physical therapy, occupational therapy, speech therapy, registered dietician, and telemedicine.

Offices:
Meridian, Caldwell, Mountain Home, and Emmett.

 

Hours:
0830-530 Monday-Friday, Nurse on-call 24 hours per day, 7 days per week.
